Disabled parking placards go through the county tax office

For a disabled parking placard or plate in Dallas County, start with TxDMV Form VTR-214. TxDMV says a doctor or health care provider completes the disability statement section of the application, and the completed application goes to the county tax office with any required payment.

Dallas County's motor vehicle FAQ gives the same local path: permanent placard applications can be mailed to the Dallas County Tax Office at 500 Elm Street, Suite 1200, or handled at a tax office location.

Renewals and replacements have their own paperwork rules, so check TxDMV and Dallas County before mailing anything. A copied form, missing ID, or missing medical statement can slow the process down.
