Disabled parking placards go through the county tax office

For a disabled parking placard or plate in Dallas County, start with TxDMV Form VTR-214. A doctor or other health care provider fills out the disability statement.

Then the form goes to the county tax office with any required payment. Dallas County gives two local options for a permanent placard: mail it to 500 Elm Street, Suite 1200, or handle it at a tax office location.

Renewals and replacements have their own rules. A copied form, missing ID, or missing medical statement can turn a simple errand into a second trip.
